15 FAA Medical Certificate Disqualifying Conditions for Pilots

Once obtained, all medical certificates expire after a set period of time and must be renewed. Alternatively, pilots who meet the BasicMed criteria have the option of transitioning to the BasicMed certification program after obtaining their initial standard FAA medical certificate.
The purpose of a medical certificate is to establish that a pilot’s physical and mental health is within the FAA’s established parameters and that they are unlikely to interfere with the pilot’s ability to safely pilot an aircraft.
Naturally, a pilot’s health status can change over time and after a new diagnosis or health event, pilots may find that they no longer meet medical standards. If a pilot’s health changes in between certifications, they are expected to notify the FAA to confirm that they still meet the criteria for holding a medical certificate.
The FAA has published a list of fifteen FAA medical exam disqualifying conditions. Pilots who are diagnosed with or experience any of these conditions may be unable to obtain or continue to hold medical certification. It is important to note that this disqualification is not set in stone.
The FAA says that “in many cases when the condition is adequately controlled, the FAA will issue medical certification contingent on periodic reports.” If you have one of these conditions, the medical division of the FAA will review your case to determine whether you qualify for a special medical issuance certification.
The FAA has a legitimate interest in prudent oversight of the safety of flight because any use of aircraft can expose innocent people and all structures to sudden and severe consequences. Medical standards are enforced to lower the risks of such events by ensuring that pilots who might have higher likelihood of reducing overall safety are monitored, restricted, or prevented from creating avoidable harm.
However … conventional medical approaches unfortunately can have undisclosed limits produced by treatments for patients. The short story: you could lose your privilege to fly because your drugs or surgery fail to bring your condition “within standards.” My 46 years of successful integrative medical approaches and hacks have been developed to produce optimal recovery available through nutrition, lifestyle, nonsurgical treatments, and even unusual uses of allowable medications – with the goal of having you regain and maintain your medical clearance to fly. One of the great misunderstandings of our era is that all movements, utterances, thoughts, emotions, memories – all of human life is basically biochemistry. Are you one of the perhaps 4,000 Americans receiving a heart transplant each year? About 90 per cent of recipients survive at least one year – and many resume quite satisfactory lives, with many living more than 10 years with their donor heart. With increasing success numbers and percentages, the prospect of a pilot returning to flight duties is higher every year. Maybe … shoot for the stars and settle for the moon? My alma mater, Stanford University, pioneered many of the advances in heart transplant surgery and recovery over more than 55 years. You might be surprised … but many of the issues encountered with heart valve replacement surgery are similar to those with total organ substitution. I have seen patients recover spectacularly well – my conclusion is that return to flight duties is an increasingly likely opportunity in the future. My clinical experience was on the kidney transplant service, but the principles and approach are similar.
As our surgical technology advances (way beyond what we did 50 years ago when I trained as a surgeon), an increasing number of pilots will be treated successfully with heart transplant. The FAA will require extensive evaluation and pilots must submit current blood chemistries and blood-clotting studies, an estimation of the prognosis for incapacitation, and an assessment and statement from a cardiologist for decision consideration.
Major concerns are to avoid production and extension of clots (causing strokes, blindness, heart attacks, and so on), unsuspected heart muscle failure, unsuspected heart rhythm irregularities, or unsuspected internal bleeding if needed anticoagulation efforts “overshoot” their intended level. [My training manual on blood clotting studies was written 54 years ago. I still endorse “old style” anticoagulation with warfarin/coumadin; but most physicians have chosen instead to use “newer” drugs … which cannot be readily “reversed” if suddenly needed … but they seem to be preferred because they require almost no continuing physician management for “correct” dosage and lifestyle/dietary adjustments.]

Are you one of the 800,000 Americans who suffers with a heart attack each year? For about 600,000, that’s their first one, often with few or no signs of an impending problem – 200,000 have already had at least one. With prompt medical attention, about 85 to 90 per cent survive. But take note: about 1 in 5 heart attacks are “silent,” meaning you don’t realize that it happened and it’s changes are discovered on later testing. Early proper treatment sets the stage for recovery to a “normal” life, so head for the hospital if any suspicion. But even more can be done for you to reduce your chances of suffering or even right then to enhance your later recovery: Do you know of a doctor who has returned a commercial captain to full flight duties after a heart attack? [Trick question: take your time to answer.]. Should you check out your possible alternatives now – for you, for your family?
Myocardial infarction, a “heart attack” sometimes called an “MI,” is simply the end result of varying causes of heart disease – seen as angina pectoris, chest pain caused by low blood flow to the heart, damaged heart pumping muscle, reduced efficiency pumping rhythms, and more. Preventive care – active planning and appropriate changes – are essential to reduce survival challenges.
Research indicates that only 50 percent of patients who experienced sudden cardiac death had known coronary heart disease before their death, suggesting that a significant portion were undiagnosed –often they experienced no obvious symptoms … or (more worrisome) they concealed possible symptoms from their physicians – including their Aviation Medical Examiner.
A study found that more than two-thirds of individuals who had a type of heart attack not caused by a blood clot also had undiagnosed heart disease, highlighting the prevalence of unrecognized conditions. Despite our many recent advances – including public distribution of “defibrillator” devices and specialized training of Emergency Medical Technicians as well as “regular people” – a study indicated that approximately 17.7 percent of individuals died within 28 days of their heart attack, with 62.3 percent of these deaths occurring instantly.
Hospitalization improves survival variably: among patients hospitalized for an initial acute myocardial infarction (AMI), in-hospital mortality rates varied based on the presence of coronary heart disease (CHD) risk factors … notably, individuals with no identified risk factors had an unadjusted in-hospital mortality rate of 14.9 percent. Long term historical data from the classic Framingham Study reported that one in five men (20 percent) who experienced their first myocardial infarction died within one year.
A substantial proportion of individuals — approximately 65 percent of men and 47 percent of women — experience a heart attack or sudden cardiac arrest as their first symptom of undiagnosed cardiovascular disease. This underscores the challenge of detecting CAD before a cardiac event occurs
These findings highlight the importance of regular health screenings and proactive management of cardiovascular risk factors to detect and address heart disease before a critical event occurs. Sadly, many conventional physicians are unaware that advanced diagnostic tests can identify people at higher risk … and are similarly unaware that reliance is misplaced on some of their current diagnostic portfolio. One study reported that the rate of myocardial infarction (heart attack) or death is rare during or shortly after a treadmill stress test, up to 1 per 2,500 tests – “rare” unless, of course, you are that “one.”
Stress tests correctly identify coronary artery disease (CAD) in only about two-thirds of individuals who actually have the condition. But only about three-quarters of people who do not have CAD are accurately reassured by stress testing. Although patients sometimes protest “too many tests,” cardiologists need to rely on a number of different ways of “looking” at your heart, trying to give you the best advice or reassurance, especially when your changes are early and minimal.
According to Harvard Health Publishing, approximately 50 percent of heart attacks occur in people with “normal” cholesterol levels. Other factors, such as smoking (current or past), unidentified or poorly controlled high blood pressure, unidentified or poorly controlled diabetes, obesity, and physical inactivity, also play crucial roles in cardiovascular health and development of devastating disease. Even in the absence of diagnosed diabetes, elevated blood sugar levels — including those within the prediabetic (“insulin resistance” or “metabolic syndrome”) range — can significantly increase the risk of cardiovascular events such as heart attacks. However, precise data on the percentage of first heart attack patients with strictly normal blood sugar levels remains limited.
A growing area of cardiovascular research is inflammation as a key contributor to heart disease – about which I was writing over 30 years ago, 10 years before the AMA Journal published details – even when cholesterol and other traditional markers look normal. A large percentage of first heart attacks occur in people with normal LDL cholesterol, but who have unrecognized vascular inflammation. Markers include High-Sensitivity C-Reactive Protein (hs-CRP), the most well-known and widely used inflammation test in cardiology. This measures very low levels of CRP, a protein produced by the liver in response to systemic inflammation. Another is Lp-PLA2 (Lipoprotein-Associated Phospholipase A2), related to vascular-specific inflammation and plaque (blockage) instability — a key cause of heart attacks.
Another marker is Myeloperoxidase (MPO), an enzyme released by white blood cells during inflammation. High MPO levels are associated with plaque erosion and rupture, even before a heart attack occurs. Interleukin-6 (IL-6) is a pro-inflammatory cytokine that stimulates CRP production and elevation is associated with increased cardiovascular and all-cause mortality. Fibrinogen is a clotting protein that also acts as an “acute-phase” reactant in inflammation, where higher levels are linked to increased blood viscosity and clot risk, contributing to heart attack risk.
Homocysteine is technically an amino acid, not an inflammatory marker — but it causes endothelial (vessel lining cells) dysfunction and promotes inflammation. Elevated levels correlate with higher cardiovascular risk, though its role is poorly understood.
Of interest, searches for “markers suggesting higher heart attack risk” rarely identify elevated levels of ferritin, an iron-transport protein found elevated more often in patients with system “acidosis” (free-radical, “rusting,” oxidation, inflammation) changes. U.S. laboratories often list levels up to “300” as “normal,” even though Salonen and colleagues reported in 1992 that “100 and below” was related to “normal” heart attack risk. Their research found that men with serum ferritin levels equal to 200 micrograms per liter had a 2.2-fold increased risk of acute myocardial infarction – and further increases showed even higher risks. These findings suggest that iron-induced oxidative stress (free radical “rusting” chemistry) could play a major role in development or worsening of arterial blockage all over.
Is the preoccupation with cholesterol misguided? Massive profits are made at every sector: pharmaceutical manufacturers (statins, other drugs: $10 to 12 billion), pharmacies (sales of such drugs and test kits: $2 to 3 billion), physicians (office visits, some testing: $5 to 7 billion), testing laboratories ($2 to 3 billion), even CME Continuing Medical Education courses ($100 to 300 million). Tens of thousands of new graduates and physicians-in-training each year are hammered with the cholesterol “myth?” In his classic confronting book reporting the scientific studies, Cholesterol Is Not the Culprit: A Guide to Preventing Heart Disease [AMAZON LINK: https://amzn.to/3FZZMqc], Dr. Fred A. Kummerow challenges the traditional view that cholesterol is the primary cause of heart disease. Drawing from his seven decades of research, Kummerow argues that dietary trans fats and oxidized fats (free radicals, acidotic chemistry), rather than cholesterol, are significant contributors to arterial blockages and cardiovascular disease.
My friend Denham Harman, M.D., developed the theory of “free-radical” oxidative damage (inflammation) in 1954, while studying radiation injury patterns to various tissues. In 1957, he offered concerns that the promotion of polyunsaturated fats (most commercial foods today!) was more likely to promote development of artery blockage disease. Research quickly advanced our understanding that free radicals can lead to oxidative stress, which contributes to aging, cancer, cardiovascular disease, impaired cellular energy production, and much more. Sadly, most physicians are unaware of these findings and clueless about what can be done to reverse damage or protect organs.
Now you have a greater understanding of why a pilot surviving a heart attack will undergo an extensive cardiovascular evaluation and must submit current blood chemistries and more, including the prognosis for incapacitation, and an assessment and statement from a cardiologist for decision consideration.

Others claim “the same” expertise in a number of areas – really?
As a National Merit and California State Scholar, I studied molecular biology at Stanford, graduating with a bachelor’s in biological sciences … and considerable experience in creative writing as well. During my pre-med studies, I assisted with research into making more effective vaccines and undertook graduate courses in immunology. Along the way, I trained as a medical laboratory technologist, receiving the highest certifications (A.S.C.P. and California State), and later I managed quality control for a Kaiser Hospital laboratory.
Half-a-dozen of my professors were Nobel Prize winners – another two dozen were members of the National Academy of Sciences – and I learned advanced literature search strategies at the National Library of Medicine, from the creator of PubMed search. My radiology training was under Harold G. Jacobsen, M. D., in the Bronx – one of the early giants in radiology.
While studying at the Case Western Reserve University School of Medicine, I produced a series of 12 color videotapes on congenital heart disease; my training in blood disorders and cancer was at the National Institutes of Health. Post-graduate studies in general surgery (Mt. Zion Hospital and Medical Center, San Francisco) and urological surgery (kidneys and bladder at The University of Texas, Texas Medical Center, including the M. D. Anderson Cancer Center) gave me surgical perspectives rarely seen in general medical practice.
To further hone my skills in research techniques, I served for 10 years on an FDA-authorized Institutional Review Board, approving applications for clinical studies … or not!
I didn’t just read about this distressing condition needlessly grounding pilots, I was one of the first to popularize understanding of “Auto-Brewery Syndrome” – “drunk without drinking” because gut microbes make alcohol then absorbed into your bloodstream – a totally controllable infection described in my 1986 book, The Yeast Syndrome!
I don’t just “boast a good line,” my career-long successes speak for themselves across so many usual and unusual conditions that steal your health, worsen your suffering, and finally take your life – no matter who claims “advanced” or whatever, there’s no substitute for experience and proven accomplishment.
I seem rather passionate about flying as well as medicine …
First of all, I found my medical training to be sadly lacking – “fighting disease” rather than “restoring better health.” So – I’ve spent a lifetime learning ways of promoting health, realizing that your body functions very much like an airplane, it does just exactly what it is supposed to do in the situation.
Second, I didn’t just watch planes flying overhead, I felt them in my bones: my Dad was a B-36 command pilot, and the Air Force base often vibrated with engine run-ups for hours before launch.
Third, reading Wolfgang Langewiesche’s outstanding classic, Stick and Rudder, during my ground school studies, I was entranced by his description of the “safety airplane” – that was the non-spinnable Ercoupe just being marketed in 1940. (Yes, the very first time I saw one, I bought it … on credit!)
Finally, I have enjoyed “basic flying” in my Ercoupes so much since 1977, I made it my mission to keep as many pilots flying safely as possible. Serving as an Aviation Medical Examiner (Senior for 12 years), I never had an application for a SODA/”waiver” ever rejected, thanks to patient recoveries with my innovative integrative “medical”/health approaches that I now share with you!
